The PreClinical Clubfoot Deformity

In 2002, I published a paper in the Journal of Bodywork and Movement Therapies describing a previously unreported embryological foot structure, the PreClinical Clubfoot Deformity, which I linked to the incomplete torsional development of the talus and calcaneus.

Etiology: Torsional development of the calcaneus and talus ends before the 9th week post-ovulation, calcaneus and talus remain in supinatus (See Figure 1).

Objective Findings:
  • PMSv between 30 and 60 millimeters
  • Weight bearing, knees extended, the feet are pronated (See Figures 2 below)

  • Standing on toes, the inner longitudinal arch is intact; standing on the heels, the ILA collapses to the ground (See Figures 3 & 4 below)

Differential Diagnosis
:
(most common requiring elimination)

(1) Rothbarts Foot
  • The Knee Bend Test facilitates the differential diagnosis between these two embryological foot structures
  • PMSv lower (typically less than 26mm) for the Rothbarts Foot
  • Treadmill Analysis
(2) Tarsal Coalition



(3) Spring Ligament Laxity (Ehlers-Danlos Syndrome and Marfan Syndrome)

(4) Mild Clubfoot Deformity
