Ossification of the iliac apophysis starts at the anterior superior iliac spine and progresses posteromedially. The iliac crest is divided into 4 quadrants. The stage of maturity is designated by the number of quadrants that have ossified. For example, Risser grade 3 indicates that 75% of the iliac crest has ossified. Risser grade 4 indicates that all four quadrants have ossified. Risser grade 5 indicates that all 4 quadrants have ossified and fused to the iliac crest.
